Working to develop and strengthen community based service systems
so families and children with special health care needs can use them
easily.
Current activities include:
- Develop Operational planning and collaboration with
community partners for easy to use services
- Champion a Medical Home Approach for Children with
Special Health Care Needs with community providers,
families and partners
- Participate on coalitions, community action groups
and existing medical home efforts to evaluate
effectiveness, barriers and solutions for community
health care systems
- Define community partner roles in early
identification, referral and follow up systems so
services are easy to use
- Engage family members and/or youth on community
task forces and work groups

How Can HCP help?
The Health Care Program for Children with Special Needs (HCP) seeks to ensure statewide access to integrated, family-centered, culturally competent, community-based programs and services. To this end, local HCP offices work closely with community residents, providers, agencies and family leaders.
