Reporting

NOTICE: The small county local health agencies contracted with HCP no longer need to submit

This table has previously been due in Jan. each year. State HCP and MCH no longer need to collect these data from each county on an annual basis.

State HCP will still send you the number of children with special health care needs (Birth-21) by Dec. 1 each year.

These data report the number of children served with care coordination that each county has documented in CHIRP by Sept. 30 each year. This data represents the work done for the previous contract year, Oct. 1 – Sept. 30 and can be used by each county as needed and/or for county reports.

Currently, there is no additional reporting required from the small county local health agencies contracted with HCP.
