What is HCP and what does HCP do?

HCP is a federally funded, state program for Children with Special Health Care Needs. 

For families …
Whether your child is a newborn or a teenager, HCP can help:

  • Evaluate your family’s needs
  • Talk to you about medical care, services, insurance and more
  • Connect you with what you need
  • Support you and introduce you to other parents and professionals who care

What are “special health care needs”?

Every child is special. All children deserve attention to their health care needs. Some children have needs that are greater than most – these are the children that HCP helps, from birth to age 21. Special health care needs can be physical, emotional or behavioral. These needs may involve conditions that last a lifetime or end with treatment. These needs may be easy to see or not seen at all. Even if you just think you may have a child with special health care needs, please call HCP to learn more.
What is a “Medical Home”?

The term “Medical Home” refers to an approach to health care.  This model insures that all   providers of a child’s care operate as a team and that families are critical participants.  A medical home approach strives to assure that health care is comprehensive, well coordinated, continuous over time, easily accessible, comprehensive and family centered.  A medical home approach is accomplished when care and treatment options are collaboratively decided and families feel included and valued.
What is “Care Coordination”?

HCP care coordination facilitates access to and the coordination of health related services for children with special health care needs.  These services may include both medical services (physical, mental and dental) and social support services. Care coordination is achieved through a comprehensive, family-center Medical Home approach.

The vision of HCP care coordination is to improve the quality of life for families by improving their ability to appropriately and effectively utilize the health care system.  HCP strives to provide referral assistance, education and guidance in coordinating health care and related community services.

What programs offer help in paying for health care services?

Child Health Plan Plus (CHP+) is a low-cost health insurance program for uninsured Colorado children from birth to age 18 whose families make too much to qualify for Medicaid, but cannot afford private insurance for their children.

Medicaid  Medicaid is a program that pays for health insurance for some Coloradans who cannot afford it. When state and federal eligibility criteria are met, Medicaid covers families with children, pregnant women, the elderly, and people with disabilities. People who are not US citizens can only get Medicaid to pay a life threatening medical emergency.

Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Early and Periodic Screening Diagnosis and Treatment (EPSDT) is a special voluntary health care program within Medicaid for children from birth to age 21. EPSDT is designed to detect and treat health problems early in children.

Supplemental Security Income (SSI) Federal income supplement program for children and adults with disabilities

Where can I find a parent support group?

Parent to Parent of Colorado   - Connecting families of sons and daughters with disabilities or special healthcare needs in communities across Colorado.

PEAK Parent Center   - In collaboration with the Colorado Department of Education, a regional mentor program with staff members across the state, both in major cities and rural locations.

Family Voices of Colorado      - A chapter of the national, grassroots organization composed of families and friends who care for and about children with special health care needs.
