The CSHCN Unit works with State and community partners to develop a system of coordinated services and supports for children with special health care needs.
This system of support is represented by six national outcomes:
1) Families will be planning partners at all levels and be satisfied with the services they receive.
2) Children with special health care needs will receive coordinated services within a Medical Home.
3) All children with special health care needs will have adequate insurance.
4) All children will be screened early and continuously for special health needs.
5) Community-based services will be organized so families can use them easily.
6) All youth with special health care needs will have the services necessary to transition to all aspects of adult life.
