Colorado Child and Adult Care Food Program

How to Get an Application for Child and Adult Care Food Program (CACFP):

If, after you read the Information Guide, you are interested in participating in the CACFP, please give our office a call at (303) 692-2330 and we will be glad to send you an application packet.

You may also complete the New Center Applicant Intake Form and fax it to:

You will be contacted within 24 hours of receipt of the fax.

*The Computer System User Authorization Form is to request the assignment of a User ID and Password. You will need this information in order to access the on-line application, training registration and claims filing for the Child and Adult Care Food Program.
