Colorado Health Disparities Strategic Plan 2008 DRAFT

The Inter-Agency Health Disparities Leadership Council will provide a formal mechanism for fellow state agencies within Colorado to develop coordinated strategies to address the antecedents to health disparities, i.e., the social determinants of health e.g., poverty, lack of education, unemployment, difficulties in accessing healthcare, mental, etc. The Council will be co-chaired by the CDPHE Chief Medical Officer.

Sample Council Functions:

• Create a shared vision and mission on addressing health disparities in targeted populations
• Determine ways to develop culturally innovative data collection strategies within their agencies and to share the data with one another without violating HIPPA regulations
• Strengthen collaborations between this department and other state agencies to meaningfully address the health disparities of communities of color
