Health Disparities Grant Program
The Colorado General Assembly recognized that although Colorado as a whole is a healthy state, racial and ethnic minorities are disproportionately impacted by disease, injury, disability and death.
In addition, it declared that it is in the best interest of the state to establish a Health Disparities Grant Program under House Bill 05-1262. The Health Disparities Grant Program is housed within the Office of Health Disparities at the Colorado Department of Public Health and Environment. The purpose of this grant program is to provide financial support for statewide initiatives that address prevention, early detection and treatment of cancer; cardiovascular disease, including diabetes and other precursors; and pulmonary diseases in underrepresented populations. The program is coordinated with other programs within the Colorado Department of Public Health Environment including the Cancer, Cardiovascular Disease and Pulmonary Disease Program, the Women’s Wellness Connection, and the State Tobacco Education and Prevention Partnership, which also are allocated funds from the tobacco excise tax. |

Training Opportunities - Statewide
- Colorado Patient Navigator Training: The Colorado Patient Navigator Training Collaborative provides training to the growing cancer patient navigator workforce in Colorado. A patient navigator is a trained professional who assists patient access medical services and obtain resources and support by reducing barriers to care. The training program has developed core competencies with input from social workers, navigators, navigator supervisors, psychologists, and an epidemiologist. An online introductory training is free and can be accessed at www.patientnavigatortraining.org.
- Community Health Worker Certificate: Community Health Workers provide information about health plans and available services and provide health education and resource referrals in a variety of settings, such as hospitals, clinics and nonprofit agencies. More than 30 health care and community-based organizations have more than 100 Community Health Workers employed by their organizations. Questions? Contact Mary Anne Nickle at maryanne.nickle@ccd.edu.
