The Uniform Waste Grease Manifest form can be downloaded using the link above or be obtained from the Department. To obtain copies of the manifest from the Department, send your name, mailing address, Transporter registration number and number of forms requested to cdphe.hmwastegrease@state.co.us . Please allow 4-6 weeks for delivery.Some Transporters may wish to pre-complete certain portions of the manifest form to include information about themselves, frequent customers and / or destination facilities. You may download and save a blank copy of the form to your hard drive using Adobe Acrobat or the free Adobe Reader Version 8 or higher. Once saved on your hard drive, you should be able to open the form to add your information, then save it as a new file. Be sure to print multiple copies of the partially completed form so that you have a copy for the generator and destination facility.

Financial Assurance Requirements for Waste Grease Transporters
These forms can be completed and submitted online using a PC only, or printed out and completed by hand for mailing to the Department. Please hit the "Submit" button only once because it takes a few minutes to process your submission.
WT-1 Waste Tire Certificate of Registration Form (complete and submit online using Adobe Acrobat or Reader) Please hit the "Submit" button only once because it takes a few minutes to process your submission. Allow 2-4 weeks to receive your certificate of registration and corresponding waste tire decals.
Waste Tire Haulers must submit a new registration application by February 1st of each year. Waste Tire Facility registration is valid for three years from the date of issuance.
