Forms and Applications
Note for Mac users: You must have Mac OS X v10.4.3 or later
and must open these forms in Adobe Acrobat or the free Adobe
Reader (version 9 or later) in order to complete and submit PDF
forms online. If you don't, the Mac OS shows PDF files
using the MAC OS X native PDF viewer, which does not support
many Adobe PDF features. The free Adobe Reader can be
downloaded at
http://get.adobe.com/reader/ . Mac users without
Acrobat or Reader will need to print the forms and submit them
by mail
or email.
