Long-Term Care

• The Medicare.gov Web site has information on long-term care, the differences between Medicare and Medicaid and how to choose and plan for long term care.
• Colorado Single Entry Point agencies provide access to long-term care information and services.
• Colorado County Departments of Human/Social Services determine financial eligibility for programs including Medicaid and provide various services for families and individuals in the local community.

• Colorado Foundation for Medical Care has information for Home Health consumers. 

Medicaid

• The U.S. Department of Health and Human Services, Centers for Medicare and Medicaid Services, Medicaid home page.

• The Colorado Department of Health Care Policy and Financing is the state agency responsible for the administration of the Medicaid program, Child Health Plan Plus and the Colorado Indigent Care program. 

• 10 CCR 2505-10 are the state rules concerning Medicaid.

• Consumer Direction: An Alternative to Agency Care. Consumer directed care is another option for older adults and people with disabilities who need in-home assistance. In consumer direction, eligible Medicaid clients and/or their family members may choose, train, supervise and schedule their home care workers. • U.S. Department of Health and Human Services, Office for Civil Rights has information about nondiscrimination requirements in programs funded by Medicare and Medicaid.

• Colorado Area Agencies on Aging are committed to improving the quality of life for older individuals. To find the agency in your area, visit the U.S. Administration on Aging website and click on Elders & Families > How to Find Help.

• Centers for Independent Living (CIL) are regional organizations that provide services to help people with disabilities stay in the community.

• The Legal Center helps individuals with disabilities live and work in the community, and is an independent public interest non-profit organization.
