File a Complaint

To file a complaint, you will need:

• Your name and a contact phone number
• The name of the agency
• A summary of your concerns
• Names of people involved in the complaint

Then:

• Complete the Home Care Colorado complaint form.
• Fax the form to (303) 753-6214 or mail it to:
  
  Health Facilities and Emergency Medical Services Division

  Denver CO 80246 

Or, with the information from the complaint form: 

• Phone the division at (303) 692-2800 in the Denver metro area or toll free statewide at 1-800-886-7689 extension 2800.

• Phone the Home Health Hotline voice mailbox at 1-800-842-8826.

• For TTY calls, phone Relay Colorado at 711.

The complaint process can vary in length depending on the priority and nature of the complaint. Contact the division at (303) 692-2800 to verify the status of your complaint.

The division cannot investigate complaints related to billing or insurance. For complaints involving billing, contact Program Integrity (link here).

Consumer Direction: An Alternative to Agency Care

Consumer directed care is another option for older adults and people with disabilities who need in-home assistance. In consumer direction, eligible Medicaid clients and/or their family members may choose, train, supervise and schedule their home care workers.
