If you are interested in opening an Assisted Living Residence, please send a completed letter of intent form to the following address:

The initial licensing packet will be mailed to you after your letter of intent is received. The following two items are required and are all part of the licensing information packet. Please complete all requirements.

• Occurrence manual
• Regulations pertaining to Assisted Living Residences
  • Chapter II - General Licensing Standards
  • Chapter VII - Assisted Living Residences
  • Chapter XXIV - Medication Administration Regulations
