• Download a Letter of Intent, fill it out and email it to cdphe.hfemsdata@state.co.us

• Receive a username and password via email within 2-3 business days

Log in using your username and password

• Enter the portal (log in here)

• The first time you log in - You will need to change your password before you get started. Remember to record your new password and ID for future visits.
