EMT Certification Application Process
Application form and guide
Be sure to affix required signatures and supporting documents,
and then photocopy the application packet before mailing to the Department.
What is a complete application?

The applicant has filled out the required personal
information and affirmed by signature that the information contained in the application
is true and complete.

The applicant has attached the required items:
-
current copy of a signed, Department-approved CPR/BLS
card (front and back required)
-
current copy of a signed, Department-approved ACLS card
(front and back required) for EMT-I and EMT-P levels
-
signed and dated affidavit form attesting to Citizenship/Residence / or lawful presence in the U.S.
-
a notarized copy of an acceptable
proof of identification:
-
Valid Colorado Driver's License

Colorado Identification Card
-
Military Identification
-
Native American Tribal Document
-
US Coast Guard Merchant Mariner Card

The Department receives a fingerprint-based criminal history
report from the Colorado and/or Federal Bureau of Investigation.
