EMT Certification Application Process

Application form and guide

Be sure to affix required signatures and supporting documents, and then photocopy the application packet before mailing to the Department.

What is a complete application?

  • The applicant has filled out the required personal information and affirmed by signature that the information contained in the application is true and complete.

  • The applicant has attached the required items:

    • current copy of a signed, Department-approved CPR/BLS card (front and back required)

    • current copy of a signed, Department-approved ACLS card (front and back  required) for EMT-I and EMT-P levels

    • signed and dated affidavit form attesting to Citizenship/Residence / or lawful presence in the U.S.

    • a notarized copy of an acceptable proof of identification:

      • Valid Colorado Driver's License

      • Colorado Identification Card

      • Military Identification

      • Native American Tribal Document

      • US Coast Guard Merchant Mariner Card

      • Other - see the Department of Revenue rules

  • The Department receives a fingerprint-based criminal history report from the Colorado and/or Federal Bureau of Investigation.
