Background:
CDPHE has purchased an automated, secure data collection and reporting system for the 2009 H1N1 influenza response and future public health emergencies. The software package and system support uses scanning technology and form recognition software to allow multiple providers from remote locations statewide to scan patient screening/consent forms into a single secure database. This will enhance the functionality of the Colorado Immunization Information System (CIIS), our statewide immunization registry, by allowing another mechanism to rapidly collect patient-level immunization data from public and private providers administering immunization. This will greatly increase the amount of immunization data CDPHE is able to collect by increasing the numbers of providers reporting data as well as reducing the burden of data entry for providers administering immunizations.
Minimum System Requirements:
Prior to installing GoScan, please make sure your computer meets the minimum system requirements below. GoScan works with any works with all existing scanning, copier, and fax hardware.
- Intel Pentium III based processor running 600 MHz or higher
- Microsoft Windows XP or later
- Microsoft .Net Framework 3.5 or above
- 100 MB free hard-drive space for complete installation
- 256 MB RAM
- VGA color or better monitor
- TWAIN compatible scanner for direct scanning
- Network connection for hot folder scanning from copier, network scanner or fax server
Installation:
The GoScan software is quick and easy to install.
Process Overview:
- Patients or personnel fill out forms from remote locations
- Forms scanned on scanner connected to computer with GoScan software
- GoScan sends all images to a central location
- Teleforms processing software picks up each image, reads each field and exports an XML file containing all form fields plus the image to a network folder
- GoImage software picks up the XML data and images and stores in a SQL database
- Data validation occurs to ensure data is being read accurately
- CDPHE staff export a full or partial set of data based on search criteria import into CIIS
Patient Forms:
The following patient screening forms were created to be used with the GoScan software. No special ink is required to print these forms and you can print them on any color paper.
