Delayed Registration of Birth

Instructions to Register a Colorado Delayed Certificate of Birth

According to the Colorado Revised Statutes (25-2-114) in conjunction with the Code of Colorado Regulations (5 C.C.R 1006-1), any person born in Colorado whose birth was not recorded at the Colorado Department of Public Health and Environment may register a ‘delayed’ certificate of birth. Following are the instructions for requesting the registration of a delayed certificate of birth.

1. Submit an Application for Certified Copy of a Birth Certificate pdf file to Vital Records to search the birth records maintained in the Office of the State Registrar. The address and ID requirements are listed on the application.

2. If no record is found, the applicant will receive a ‘Letter of No Record’ and an Application to Register a Delayed Birth Certificate, which will include information about the registration process.

3. Eligible parties* interested in requesting the registration of  a ‘Delayed Birth Certificate’ are to complete and submit the Application to Register a Delayed Birth Certificate with the $20 processing fee and the required supporting documentation to the following:

The supporting documents pdf file will be reviewed and sent for verification to the issuing agency.  This part of the process may be lengthy and can take up to one (1) year depending upon the turnaround processing time between Vital Records and the issuing agency.

*Eligible parties who may request the registration of a Delayed Birth Certificate:

(a) Registrant (person who’s birth was not registered)

(b) One of the parents of the registrant, or

(c) The guardian of the registrant, or

(d) Any older person in his immediate family having personal knowledge of the facts of birth.

4. If the minimum requirements are met a delayed standard registration form will be prepared with the supporting documents pdf file listed on the face of the record and sent to the applicant. This is not a birth certificate. The applicant will review for accuracy and will obtain the signature of a Public Notary verifying the authenticity of the information.  The applicant will then return the notarized form to Vital Records to be registered and issued on state certified paper.

If documentary evidence is deficient or invalid, the certificate shall not be registered in accordance to the Colorado Revised Statutes (25-2-114).
