Correct a Colorado birth or death certificateSome changes to a Colorado birth certificate will require a legal name change in the county of your residence. On occasion, other types of court orders such as certified copies of divorce decree are necessary. See page 2 of the forms below for more information. Generally you will need a legal name change if the child is over one year of age and the new surname does not conform to your legal surname or you do not have adequate documentary evidence to support the name change. These forms cannot be used to add/remove the biological father to/from the birth certificate. For more information go to our Frequently asked questions page.
