Due to recent legislation, Colorado "heirloom" birth and marriage certificates are now available for purchase from the Vital Records Section of the Colorado Department of Public Health and Environment. These commemorative certificates are suitable for framing. Ordering Information/FormsYou may order in person, by fax, by mail, or online. The Colorado State Vital Records office does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express, Discover, Mastercard or VISA.

The Vital Records Section of the Colorado Department of Public Health and Environment maintains official documentation of vital events in our state. Certified copies of these legal documents are necessary or helpful for many purposes, such as:
Marriage & Dissolution Verifications Since 2003, the Office of the State Registrar of Vital Statistics has operated a web site where the general public may access information on marriages and dissolutions of marriage. As the issue of identity theft has grown, concerns have been expressed regarding the potential for misuse of this information. As a result, these records are now treated confidential consistent with other vital records in this state. Agencies that believe they have a direct and tangible interest in being granted access to researching these records need apply to the Office of the State Registrar. An agency application needs to be submitted to the Colorado Dept. of Health and Public Environment Registrars Office. Individuals needing verification of a Marriage or Dissolution need to apply to the State Vital Records office.
