Permit
Application
Form:
Submit
this
form
to
obtain
an
air
permit
for
a
new
source
of
air
emissions,
to
modify
an
existing
air
permit,
or
to
request
an
administrative
amendment
to
an
existing
air
permit.
(Administrative
permit
amendments
include
transfer
of
ownership,
company
name
change,
or
other
minor
permit
corrections
such
as
typographical
errors.)
The
Permit
Application
Form
is
not
required
for
routine
(5-year)
APEN
renewals
when
there
is
no
change
to
an
existing
permit.
The
Permit
Application
Form
should
always
be
submitted
in
conjunction
with
a
General
APEN
or
Specialty
APEN,
appropriate
Supplemental
Forms
or
Notifications,
and
the
required
filing
fee.

Notice
of
Start
Up
A
Notice
of
Start-Up
must
be
submitted
to
the
Division
no later than
30
days
after commencement of operation of the emissions source.
Therefore,
if
you
wish
to
begin
operating
a
source
upon
receipt
of
the
Initial
Approval
Permit,
please
plan
ahead
and
submit
a
Notice
of
Start-Up
along
with
your
initial
permit
application.
General
Air
Pollutant
Emission
Notice (APEN) form

The
General
APEN
is
a
generic
form
used
to
obtain
an
air
permit
for
a
new
source
of
air
emissions,
to
modify
an
existing
air
permit,
to
request
an
administrative
amendment
to
an
existing
air
permit,
or
to
complete
a
routine
(5-year)
APEN
renewal
even
when
there
is
no
change
to
an
existing
permit.
When
a
"Specialty
APEN"
exists
for
a
given
source,
the
Specialty
APEN
should
be
used
in
lieu
of
the
General
APEN.
