If you have more than one facility, complete the certification checklist for the facility noted on the postcard (or postcards) you received.  Please print a copy of your completed form for your records before hitting the "Submit" button.  Once you hit the Submit button, you will automatically be re-directed to a related web page and will not be able to print your form.  Your certification is not complete until you have hit the Submit button on the form. 

Please use the links as provided in the checklist.  DO NOT use your browser "Back" button as you will be taken out of the checklist and your answers will not be saved.

This form must be completed by an employee that is directly involved
with hazardous waste management at your facility.

If you need to make any changes to your hazardous waste notification form, such as a change in ownership, name of facility, contact information, change in location, or change in your generator status, please complete and return an updated Colorado Notification Form.
