If you have more than one facility, complete the certification checklist for the facility noted on the postcard (or postcards) you received.  Please print a copy of your completed form for your records before hitting the "Submit" button.  Once you hit the Submit button, you will automatically be re-directed to a related web page and will not be able to print your form.  Your certification is not complete until you have hit the Submit button on the form.

This form must be completed by an employee that is directly involved
with hazardous waste management at your facility.

The next small quantity generator self-certifications will not be due until mid-2012.


Surface Coating Small Quantity Generator Checklist - if any part of your business is to conduct painting and/or body repair on vehicles, such as automobiles, motorcycles, trucks, buses, truck toppers, truck trailers, utility trailers, camper trailers, truck-mounted campers, other recreational vehicles, aircraft, boats, or railcars.
